BANKRUPTCY CLIENT CHECKLIST

1. You must complete a credit counseling course. We will enroll you in the course and provide you with a username and password. The cost is $40.00 and the website is: www.bkcert.com
2. You must provide us with full copies of your Federal Tax Returns- Forms 1040 for the last two years for a Chapter 7 and four years for a Chapter 13. We cannot file your case without copies of these returns.
3. You must provide us with the last six (6) months of all your paystubs from any and all jobs you or a spouse in your household has held even if your spouse is NOT filing for bankruptcy. The paystubs must be complete and show all deductions. If you are not employed we must see Social Security statements, disability statements, unemployment stubs, and pension statements.
4. You must provide us with a copy of all bills you have including:
• Credit card bills
• Medical bills
• Letters from collection agencies
• If you owe the IRS or the State of IL taxes you must provide tax bills
5. If you own a home we must see a recent mortgage statement
• If your home is in foreclosure we must see the foreclosure papers
• You must provide us with a recent property tax bill if you pay your property taxes directly
6. If you own an automobile that you are financing we must see a recent statement from the automobile finance company showing the most recent payoff balance and the monthly payment amount.
7. You must provide us with a monthly budget showing us your average monthly expenses.

For existing clients, uou must provide our office with the above information before you come in to our office and sign your final paperwork to have your case filed.
